(Abstract) 입력
Objective: It is known that the restenosis after coronary stenting is related to inflammatory reaction. However, few reports have described about the relation between neointima formation and inflammation in the in-stent restneosis.
Methods: Twenty two coronary stents were deployed in porcine coronary arteries. Follow-up coronary angiogram and the histopathologic assessments of stented porcine coronary was performed at 14 days (Group I, n=8), 28 days (Group II, n=8), 56 days (Group III, n=6) after stenting.
Results: The neointima area was 0.48±}0.16 mm2 in Group I, 1.36±}0.53 mm2 in Group II, 2.17±}0.11 mm2 in Group III and the histopathologic restenosis was 17.4±}6.2%, 47.0±}18.0%, 50.7±}22.8% in each group. The neointima formation was rich especially between 14 days and 28 days. The injury score of stented porcine coronary arteries was similar in three groups (1.26±}0.61, 1.34±} 0.70, 1.28±}0.91). The percentage of inflammatory cells per total number of cells in neointima was 20.24±}5.76% in Group I, 15.32 ±}7.43% in Group II , 9.07±}5.20% in Group III. The number of inflammatory cells in media was small in Group I, but increased in Group II and decreased in Group III. The fibrinoid material associated with inflammatory cells was observed all arteries of Group I, few in Group II, but no in Group III. Granulation was observed 1 case in Group II, 3 cases in Group III, but no in Group I.
Conclusion: Neointima formation after coronary stenting is rich between 2 weeks and 4 weeks through all vascular layers including media. These data suggest that the inflammatory reaction during this period plays an important role in neointimal formation after coronary stenting.
